The Uniform Hazardous Waste Manifest is the key component of this system. This form is required by both the Department of Transportation and the EPA. All waste generators who transport, or offer for transport, hazardous waste for offsite treatment, recycling, storage, or disposal must prepare a Uniform Hazardous Waste Manifest.

The manifest contains the following information:

• Type and quantity of the waste being transported
• Instructions for handling the waste
• Signature lines for all parties involved. (Each party that handles the waste signs the manifest and keeps a copy for themselves.)

Once the waste reaches its destination, the receiving facility returns a signed copy of the manifest to the generator to confirm that the waste has been received by the designated facility.
